Where do I find my pathology referral?

You can find your referral by either:
Logging into the Everlab App and clicking on your pathology appointment.
Finding it attached on the calendar invite for your pathology appointment.
For your initial Pathology appointment will be two forms attached: one for Medicare tests and one for corporate requests. If you have any questions, please contact us at (+61) 489 078 867 or email us at support@everlab.com.au.
Please print out both forms if possible and bring them to your pathology provider. If you are unable to bring a physical copy, have the referral ready on device to show your provider.
