What do I need to bring to my pathology test?

You’ll need to bring your pathology referral with you to your test.
For Initial Diagnostics, the referral is 2 pages.
For Follow Up Diagnostic Review (Protocol Program members only), it is 1 page.
Where to Find Your Referral
You can find your referral in your Everlab Dashboard or App under the Pathology appointment banner.
We recommend bringing a printed copy of the referral with you to the test.
If you can’t print it:
Some pathology providers accept referrals via email.
You can contact your selected provider to confirm their preferred process and email address.
